You will be assessed by our team and one of our nurses may call you to discuss the best way to get you the care you require.

Requesting a testing kit

 

Testing for chlamydia is quick and simple - requiring only a urine sample for men/those with a penis or a self-taken vaginal swab for women/those with a vagina. If you're aged over 16 and you live in North Yorkshire you can use the form below to request a postal kit to be sent to you free of charge. Kits are sent in a plain, discreet envelope. If you are over 16 years old and want to be checked for chlamydia, gonorrhoea, HIV and syphilis in one test you can order one here. If you are under 16 you can still get tested but just need to talk with a nurse, doctor or youth worker first - call us on 01904 725440 or contact us for more details.

The postal test request below is only for chlamydia and does not look for other STIs. The vaginal swabs and urine tests won't detect rectal or throat infections. If you're concerned about other infections, our sexual health clinics will be able to help.

PLEASE use your own name and date of birth on all tests (to avoid confusion) but feel free to use an address of your choice when you request the kit (as long as it is in North Yorkshire or York)
